Patients suffering from multiple traumas, status post joint replacement, or generalized weakness from long hospital stay or severe illness will be evaluated by our rehab consultation team for inpatient rehab admission.  

The physician team, therapists, nurse, psychologists and clinical social worker, will evaluate and treat patient’s medical illness and physical disability. The team will work with patient and family to establish the rehab goal to maximize the patient’s mobility and ADL skills. 

The discharge plan will be carefully evaluated and community rehab resources will also be explored for continuation of the rehabilitation program.  The patient will follow up with the rehab-attending physician in order to monitor the function recovery after discharge.
