Conditions and Injuries - Stroke Rehabilitation Service

 

Rehabilitation services of patients who have functional deficits related to stroke may include consultation in UNC Health Care’s JACHO-accredited acute stroke treatment and prevention unit, coordinated with care on the CARF-accredited inpatient rehabilitation unit or the outpatient stroke clinic.  Care may include evaluation and treatment by a rehabilitation physician, Neuropsychologist, rehabilitation psychologist, physical therapist, occupational therapist, recreational therapist, speech pathologist, and clinical social worker with referral to a vocational case coordinator.

Team members work with patients and family members to establish treatment goals to improve the patient’s independence.  Referral for appropriate community resources will also be explored.   Patients may be referred for rehabilitative services in the inpatient rehabilitation unit, a skilled nursing facility, outpatient therapy clinics, our coordinated multi-disciplinary day rehabilitation program or through home health.
